You're Doc Tommy. You run a 24-hour dental office in North Jersey — real patients, real crowns, real charts, work you're actually good at. It's also the cover. The patient schedule launders the Family's heat, the back counter runs a sportsbook and a loan sheet, and the safe holds two kinds of money that must never be seen in the same room.

Don Salvatore “Tessio” DeAngelo erased your debt once. In exchange you run his book and his loan outfit. That isn't a job you quit.

The office never closes.

Book the Day

A day is a handful of calendar blocks and a finite pool of effort. Every appointment and every favor spends both. A meeting you didn't book books you.

Keep the Chair Full

The schedule is the cover. A thin day raises the heat; a full one buys it back. The heat never goes all the way down.

Keep the Money Moving

Clean money is spendable and explicable. Dirty money spends fine inside the operation — sitting on it is the cost.

Two Sets of Books

One the Don sees, and one he doesn't. His son, who pays nothing. New York on the other line, being very polite.
